High-sensitivity troponin I in hospitalized and ambulatory patients with heart failure with preserved ejection fraction: Insights from the Heart Failure Clinical Research Network

Fudim M, et al. - In this post hoc analysis including 139 hospitalized and 212 ambulatory patients with a preserved ejection fraction (HFpEF) and high-sensitivity troponin I (hs-TnI) measured at baseline, researchers determined the prevalence as well as association of high-sensitivity troponin with cardiac structure and outcomes. Patients hospitalized with acute HFpEF were enrolled in DOSE (Diuretic Optimization Strategies Evaluation) and CARRESS-HF (Cardiorenal Rescue Study in Acute Decompensated Heart Failure), ambulatory patients with HFpEF in RELAX (Phosphodiesterase-5 Inhibition to Improve Clinical Status and Exercise Capacity in Heart Failure With Preserved Ejection Fraction). Findings revealed a common prevalence of elevated hs-TnI among both patient populations, 86% in hospitalized patients and 53% among ambulatory patients. Also, older subjects with worse left ventricular hypertrophy and diastolic dysfunction were seen to have a hs-TnI above the median. Worse cardiac structure and increased risk of adverse events were observed in relation to increased hs-TnI levels.
