High-sensitivity C-reactive protein in HIV care: Tuberculosis diagnosis and short-term mortality in a cohort of Kenyan HIV patients in the DREAM programme

Ciccacci F, Welu B, Ndoi H, et al. - Researchers conducted the IDEA-TB Study enrolling HIV-positive adult patients attending three DREAM centres in Kenya who were suspected of having TB with the aim to determine the role of high-sensitivity C-reactive protein (HS-CRP) in TB diagnosis and the prediction of mortality in HIV-positive patients. Enrollment of a total of 574 patients was done. Diagnosis of TB in HIV-positive patients remains challenging. Findings revealed association of elevated HS-CRP levels with TB test results in HIV-positive Kenyan patients. They indicated an independent association of malnutrition and raised HS-CRP with TB in HIV-positive patients. Short-term mortality was observed, especially in TB-negative patients, in correlation with the combination of malnutrition and raised HS-CRP. Based on the findings, they suggest a possible role of HS-CRP in HIV care services as a predictor of TB infection and short-term mortality.
