Frequency and impact of hyponatremia on all-cause mortality in patients with aortic stenosis

Ramberg E, Greve AM, Berg RMG, et al. - Researchers investigated whether hyponatremia heralds poor prognosis among patients having asymptomatic aortic stenosis (AS), and if AS in itself is related to raised risk of hyponatremia. Participants were 1,677 people that had and annual plasma sodium measurements in the SEAS (Simvastatin and Ezetimibe in Aortic Stenosis) trial; randomization of 1,873 asymptomatic patients with mild-moderate AS to simvastatin/ezetimibe combination vs placebo was performed. The primary endpoint was all-cause death and the secondary outcome was incident hyponatremia (P-Na + <137 mmol/L). Findings revealed increased mortality in correlation with both prevalent and incident hyponatremia in patients with AS. For hyponatremia, the estimated prevalence was around 4% and the incidence approximately 2% per year, which was comparable to that of older adults without AS.
