Facility-level association of preoperative stress testing and postoperative adverse cardiac events

Valle JA, et al. – Given that patient-level analyses of preoperative stress testing and outcomes are limited by case mix and selection bias, researchers conducted this study to determine facility-level rates of preoperative stress testing for non-cardiac surgery and to examine the link between facility-level preoperative stress testing and postoperative major adverse cardiac events (MACE; death, myocardial infarction, or revascularization within 30 days). Participants included patients undergoing non-cardiac surgery within 2 years of percutaneous coronary intervention in the Veterans Affairs Health Care System (2004-2011). Facility-level rates of preoperative stress testing were found to show substantial variation. No better postoperative outcomes were reported in relation to facilities with higher rates of preoperative stress testing.
