External validation of the ORBIT Bleeding Score and the HAS-BLED Score in non-valvular atrial fibrillation patients using direct oral anticoagulants (Asian data from the DIRECT registry)

Mori N, et al. - A single-center prospective observational registry of NVAF cases treated with direct oral anticoagulants (DOACs) ie, the DIRECT registry, was carried out by the researchers in order to externally verify the ORBIT bleeding score and the HAS-BLED score in a large pooled real-world Asian population with non-valvular atrial fibrillation (NVAF) using DOACs. In 2,216 subjects with NVAF using DOACs, the predictive and discriminative performance of the ORBIT bleeding and the HAS-BLED scores for major bleeding was evaluated. After a median follow-up of 315 days, the overall incidence of major bleeding was estimated to be 4.2%. The ORBIT bleeding and the HAS-BLED scores both had modest discrimination skills to recognize those who had bled to those had not. Similar predictive performance when contrasted with the HAS-BLED score was exhibited by the calibration plots of the ORBIT bleeding score. Hence, modest discriminative performance and similar predictive performance for major bleeding was demonstrated by the ORBIT bleeding score and the HAS-BLED score in the real-world of the NVAF people with DOACs.
