Expansion of transcatheter aortic valve replacement in the United States

Mentias A, Sarrazin MV, Desai M, et al. - To determine patterns of diffusion of TAVR in the United States (U.S.) as well as its association with racial differences in TAVR utilization, researchers performed this investigation using the Medicare database. Experts identified TAVR hospitals in the continental U.S. from 2012-201 and mapped them to Hospital Referral Regions (HRR). They noted an increase in the number of TAVR hospitals from 230 in 2012 to 540 in 2017. An increase in the proportion of the U.S. population residing < 100 miles from nearest TAVR hospital was noted, which was from 89.3% in 2012 to 94.5% in 2017. For all racial and ethnic subgroups, an improvement was seen in geographic access. Within a HRR, higher odds of opening a new TAVR program were observed among teaching hospitals and hospital bed size. Overall, an increase in geographic coverage for all racial subgroups has been noted along with expansion of TAVR programs in the U.S. There is a requirement for further study to ascertain reasons for TAVR underutilization in Blacks and Hispanics.
