Epidemiology of qSOFA criteria in undifferentiated patients and association with suspected infection and sepsis

Anand V, et al. - In view of the controversial role of Quick Sequential Organ Failure Assessment (qSOFA) criteria in sepsis screening and management, particularly as they were derived only in patients with suspected infection, researchers investigated the epidemiology and prognostic value of qSOFA in undifferentiated subjects. Within 1 day of admission, patients with ≥2 qSOFA criteria in all adults admitted to 85 U.S. hospitals from 2012-2015 were identified and evaluated for suspected infection using Sepsis-3 criteria using clinical cultures and antibiotics and sepsis. In patients with and without suspected infection using regression models, the discrimination of qSOFA for in-hospital mortality was also examined. Investigators found that only 1 in 3 patients suspected of being infected with qSOFA-positive on admission and 1 in 6 patients with sepsis. Findings revealed that qSOFA also has low sensitivity to identify suspected infection and sepsis and is not specific to infection for its prognostic significance. For sepsis screening and risk stratification, more sensitive and specific tools are needed.
