Effectiveness of pre-entry active tuberculosis and post-entry latent tuberculosis screening in new entrants to the UK: A retrospective, population-based cohort study

Berrocal-Almanza LC, Harris R, Lalor MK, et al. - Researchers examined new-entrant migrants to the UK to assess if screening for active tuberculosis before entry to the UK and for latent tuberculosis infection (LTBI) post-entry for reduction of tuberculosis is efficacious. In addition, they investigated how access to primary care affects tuberculosis incidence in this population. In this retrospective, population-based cohort study, migrants from 66 countries who were negative for active tuberculosis at pre-entry screening between Jan 1, 2011, and Dec 31, 2014, and eligible for LTBI screening were included. Using record linkage, they traced the first contact of the patients with primary care, their uptake of LTBI screening, and development of active tuberculosis in England, Wales, and Northern Ireland. Participants were 224,234 migrants who were screened for active tuberculosis before entry to the UK and 118,738 migrants (control group) who were not. During the cohort time, they identified 1,828 tuberculosis cases, of which 31 were prevalent. Observations revealed a lower tuberculosis incidence in new-entrant migrants in strong and independent correlation with pre-entry tuberculosis screening, early primary care registration, and LTBI screening.
