Direct current cardioversion of atrial arrhythmias in adults with cardiac amyloidosis

El-Am EA, et al. - Given that arrhythmias, conduction abnormalities, and intracardiac thrombus are commonly seen in patients with cardiac amyloidosis (CA), researchers assessed direct-current cardioversion (DCCV) procedural outcomes in patients with CA. Participants were CA patients (n = 58, mean age 69 ± 9 years, 81% male) scheduled for DCCV for atrial arrhythmias, matched 2:1 with control patients by age, sex, type of atrial arrhythmia, and date of DCCV. Findings revealed that a significantly high cancellation rate was observed in patients with CA undergoing DCCV. This was primarily attributed to a high incidence of intracardiac thrombus, observed even among patients who received adequate anticoagulation. Despite a high success rate of restoring sinus rhythm, CA patients vs control patients were found to develop tachyarrhythmias and bradyarrhythmias significantly more frequently, complicating DCCV.
