Diastolic pulmonary gradient as a predictor of right ventricular failure after left ventricular assist device implantation

Alnsasra H, Asleh R, Schettle SD, et al. - Researchers examined the value of preleft ventricular assist device (LVAD) diastolic pulmonary gradient (DPG) for predicting survival or right ventricular (RV) failure post-LVAD. Two hundred sixty-eight patients who underwent right heart catheterization before LVAD implantation and had pulmonary hypertension because of left heart disease were retrospectively reviewed. They dichotomized patients using DPG ≥ 7 mm Hg, pulmonary vascular resistance (PVR) ≥ 3 mm Hg, or transpulmonary gradient ≥ 12 mm Hg. Outcomes reveal a correlation of elevated DPG with RV failure but not with survival, while elevated PVR was identified as predictive of mortality post-LVAD implantation among patients with pulmonary hypertension because of left heart disease requiring LVAD support.
