Development of a model to predict prostate cancer at the apex (PCAP model) in patients undergoing robot-assisted radical prostatectomy

Cumarasamy S, Martini A, Falagario UG, et al. - A retrospective analysis of 459 patients who underwent a robotic-assisted radical prostatectomy (RALP) between January 2016 and September 2017 was conducted by the experts in order to develop a model on the basis of the preoperative variables to prognosticate apical prostate cancer. Significant apical pathology (SAP) was noted in a total of 121 patients. All apex-specific variables examined appeared as predictors of SAP, on univariable analysis. On multivariable analysis, significant predictors of SAP were PSA and apical prostate imaging reporting and data system (PI-RADS) score > 3. Of the model, the AUC was 0.722. In conclusion, patients with PI-RADS 3, 4 or 5 lesions at the apex were threefold as more prone to have true SAP in comparison with those who have PI-RADS < 3 or negative mpMRI former to undergoing RALP.
