Complete vs simplified Selvester QRS score for infarct severity assessment in ST-elevation myocardial infarction

Tiller C, Reindl M, Reinstadler SJ, et al. - Researchers investigated the comparative efficacy of complete vs simplified Selvester QRS score in the prediction of myocardial and microvascular injury assessed by cardiac magnetic resonance (CMR) imaging in patients with acute ST-elevation myocardial infarction (STEMI). They conducted a prospective observational study including 201 revascularized STEMI patients. The complete and simplified QRS scores were determined via performing electrocardiography at a median of 2 (interquartile range 1–4) days after the index event. They determined acute and chronic infarct size (IS) as well as microvascular obstruction by undertaking CMR within 1 week and 4 months thereafter. Outcomes revealed comparable efficacy of complete and simplified QRS score for the prediction of acute and chronic myocardial as well as microvascular damage in reperfused STEMI. However, relative to peak hs-cTnT, the clinical reference method for IS estimation, both QRS scoring systems had inferior predictive validity.
