Comparison of a novel anterior-only approach and the conventional posterior–anterior approach for cervical facet dislocation: A retrospective study

Liu K, et al. - Ninety-three subjects with lower cervical facet dislocations were enrolled in the study and divided into two groups (63 conventional subjects who registered between January 2014 and December 2017 in the anterior-only approach group, and 30 conventional patients who registered between January 2012 and December 2013 in the posterior-anterior approach group) by the researchers contrasted the surgical trauma and clinical results of a novel anterior-only approach and vs the conventional posterior-anterior approach in order to get an optimal method to treat lower cervical facet dislocation. By the two techniques, every case in the anterior-only approach group was successfully decreased. In the posterior-anterior approach group, the mean operation time was significantly prolonged in comparison to those in the anterior-only approach group. In contrast to those in the anterior-only approach group, the mean blood loss during the surgery and the number of fixed segments in the posterior-anterior approach group were markedly bigger. In both groups after a 12-month follow-up, a 100% fusion rate was noted. No important variations between the two groups regarding the betterment in the ASIA grade or JOA score was observed. Hence, the novel anterior-only approach with two reduction techniques, including Caspar pins kyphotic paramedian distraction and anterior facetectomy, attained a 100% reduction success rate and caused less surgical trauma, in comparison to those with the conventional posterior-anterior approach, symbolizing that this method could be suggested as an alternative for lower cervical facet dislocation.
