Comorbidities and the decision to undergo or forego destination therapy left ventricular assist device implantation: An analysis from the DECIDE-LVAD study

Warraich HJ, et al. - In a stepped-wedge multicenter trial (DECIDE-LVAD), researchers examined the link between comorbidities, often seen in patients considering destination therapy left ventricular assist devices (DT LVAD), and post-decision outcomes. This study included patients considering LVADs, in whom comorbidities were recorded using INTERMACS protocol. They found that increased decision conflict, stress, and struggle with illness were experienced by patients considering LVAD implantation who have comorbidities. At 6-month follow up, worse decision regret, depression, stress and acceptance of illness were observed in patients with pulmonary hypertension vs those without pulmonary hypertension, however, no difference was evident in patients with chronic renal disease or high body mass index.
