Clinical significance of mirror lesions in lower extremity arterial disease

Kiesz RS, Góra B, Kolarczyk-Haczyk A, et al. - Since no prior study has assessed the clinical importance of symmetrical lesion patterns, especially the coexistence of same-level significant plaques in both lower extremities (“mirror lesions”), researchers performed a single-facility, primary data analysis to examine the clinical relevance of bilateral peripheral arterial disease (PAD) patterns. Participants in the study were 225 individuals with symptomatic PAD. Even in cases with negative screening outcomes, individuals with known peripheral arterial disease need to have both limbs fully assessed and monitored. Mirror angiographic imaging is common and frequently accompanied by claudication symptoms. Unilateral ischemia is a significant risk factor for contralateral illness. Critical limb ischemia (CLI) patients had a history of resting pain, ulceration/gangrene, or previous amputation of the contralateral limb. CLI patients are at high risk for occlusive nonindex limb lesions.
