Clinical impact of routine complete blood counts following total knee arthroplasty

Howell EP, et al. - In this study, 484 individuals were retrospectively examined to evaluate the efficiency and cost-effectiveness of complete blood count (CBC) tests after total knee arthroplasty (TKA). They recorded the need for transfusion following TKA in 5.2% of candidates. They observed significantly lower pre-operative hemoglobin in subjects requiring transfusion when compared to patients who did not require transfusion (11.47 g/dL vs 13.58 g/dL). They recorded 5.2 times higher risk of transfusion in cases with preoperative anemia. They found fetching of a total cost of $144,773.80 in associated hospital charges ($316.10 per patient) with an average of 2.9 CBC tests collected per patient who did not receive medical intervention. They did not find routine post-operative CBCs valuable for subjects with normal pre-operative hemoglobin who endured tranexamic acid (TXA) during TKA performed under tourniquet.
