Circulation of non-polio enteroviruses in 24 EU and EEA countries between 2015 and 2017: A retrospective surveillance study

Bubba L, et al. - As in most countries in the EU and European Economic Area (EEA) region, non-polio enterovirus infections are not notifiable, and there is substantial variation in surveillance between countries, researchers sought to assess the current epidemiological situation and need for standardizing surveillance across these regions. From representatives in all 31 EU and EEA countries, they requested for data on any enterovirus detected between Jan 1, 2015, and Dec 31, 2017, through national enterovirus reference laboratories. They identified this study as the largest investigation of enterovirus circulation in EU and EEA countries and recognized the availability of non-polio enterovirus data in the region. Among children younger than 5 years, they identified 9,914 (66%) of 14,999 enterovirus infections with information about age; among 7,139 individuals with reported symptoms, 3,197 (45%) had neurological symptoms. Non-specific fever (in 1,607 [23%] patients), respiratory symptoms (in 1,197 [17%] patients), hand, foot, and mouth disease (in 528 [7% patients), and myocarditis (in 39 [1%] patients) were the other reported symptoms. Temporally, 68 deaths were linked with enterovirus infection. They performed typing for 11,559 (67%) of 17,136 specimens unveiling 66 enterovirus types; the most frequently detected enterovirus type was Coxsackievirus A6 (in 1,556 [13%] of 11,559 typed enteroviruses). Among 448 patients with coxsackievirus A6 infection with available clinical data, 292 (65%) presented with hand, foot, and mouth disease. The second most frequently detected enterovirus type was Echovirus 30, representing 1,412 (12%) of 11,559 typed enteroviruses; among 467 individuals with echovirus 30 infection with available clinical data, 384 (82%) had neurological symptoms. As per sequences available from 18 countries, there is a circulation of newly emerging strains of enterovirus A71 and enterovirus D68.
