CHA2DS2-VASc score and stroke prediction in atrial fibrillation in whites, blacks, and Hispanics

Chen LY, et al. - Using data from Optum Clinformatics, a large administrative claims database, researchers compared the ischemic stroke (IS) or transient ischemic attack (TIA) incidence by CHA2DS2-VASc in black and Hispanic vs white patients with atrial fibrillation, compared the predictive ability of CHA2DS2-VASc to IS or TIA in black and Hispanic vs white patients, and determined improvement in the predictive ability of CHA2DS2-VASc score from addition of race/ethnicity. Study participants included 267,419 patients with atrial fibrillation (mean age, 73.1 years; 84.2% white, 8.5% black, 7.3% Hispanic) enrolled in commercial and Medicare Advantage health plans from 2009-2015. According to results, the predictive ability of CHA2DS2-VASc in atrial fibrillation for IS or TIA is comparable between white, black and Hispanic individuals, and can, therefore, be used in the latter two groups. The race/ethnicity addition to the CHA2DS2-VASc, however, does not improve its predictive ability.
