Brachial pulse pressure in acute heart failure- Results of the Heart Failure Registry

Bonapace S, Rossi A, Laroche C, et al. - Researchers focused on the independent prognostic influence of pulse pressure (PP) in acute heart failure (HF), especially across the left ventricular ejection fraction (EF) phenotypes, and determined the potential contribution of PP in outlining the individual phenotypes. For this purpose, they performed a prospective assessment of 4,314 patients admitted for acute HF classified by EF and stratified by their PP level on admission, to determine 1-year death and rehospitalization in these individuals. Findings revealed an almost linear inverse link between mortality and PP, partially mediated by systolic blood pressure (SBP), in acute HF with reduced (< 40%) EF. The presence of a J-shaped link between mortality and PP was noted in HF with preserved (≥ 50%) EF with a better prognosis at the nadir. The possible clinical usefulness of a combination of PP > 60 mmHg with SBP > 140 mmHg was suggested as a marker of a preserved left ventricular EF.
