Bicuspid aortic valve morphology and outcomes after transcatheter aortic valve replacement

Yoon SH, Kim WK, Dhoble A, et al. - Researchers examined the association of bicuspid aortic valve (BAV) morphology and results of transcatheter aortic valve replacement (TAVR) with the new-generation devices. They used the international multicenter BAV TAVR registry to identify patients with BAV verified by central core laboratory computed tomography analysis. Overall 1,034 BAV patients who underwent TAVR with contemporary devices (n = 740 with Sapien 3; n = 188 with Evolut R/Pro; n = 106 with others) were analyzed. The participants had a mean age of 74.7 years and Society of Thoracic Surgeons score of 3.7%. All-cause 30-day, 1-year, and 2-year mortality was estimated to be 2.0%, 6.7%, and 12.5%, respectively. Findings demonstrated that TAVR outcomes in bicuspid aortic stenosis relied on valve morphology. An increased risk of procedural complications and midterm mortality was reported in relation to calcified raphe and excess leaflet calcification.
