Association of race with disease expression and clinical outcomes among patients with hypertrophic cardiomyopathy

JAMA Dec 19, 2019

Eberly LA, Day SM, Ashley EA, et al. - Researchers examined how race influences disease expression, care provision, and clinical outcomes among patients with hypertrophic cardiomyopathy. From the US-based sites of the Sarcomeric Human Cardiomyopathy Registry from 1989 through 2018, they analyzed 2,467 patients with hypertrophic cardiomyopathy; of these, 205 (8.3%) were black (130 male [63.4%]; mean age, 40.0 [18.6] years) and 2,262 (91.7%) were white (1,351 male [59.7%]; mean age, 45.5 [20.5] years). Among black patients vs white patients, hypertrophic cardiomyopathy was diagnosed at a younger age; further, black patients less frequently had sarcomere mutations, and had worse symptoms. The race was also correlated with inequities in healthcare access and delivery, with black patients with hypertrophic cardiomyopathy having lower rates of genetic testing and invasive septal reduction therapy.
