Association of isolated diastolic hypertension as defined by the 2017 ACC/AHA blood pressure guideline with incident cardiovascular outcomes

McEvoy JW, Daya M, Rahman F, et al. – Researchers compared the prevalence of isolated diastolic hypertension (IDH) in the United States as defined by the 2017 American College of Cardiology (ACC)/American Heart Association (AHA) blood pressure guideline with the prevalence of IDH based on the 2003 Joint National Committee (JNC7) definitions. They also characterized cross-sectional and longitudinal associations of IDH with outcomes. To do so, they conducted cross-sectional analyses of the National Health and Nutrition Examination Survey (NHANES 2013-2016) and longitudinal analyses of the Atherosclerosis Risk in Communities (ARIC) Study (baseline 1990-1992, with follow-up through December 31, 2017). The study sample consisted of approximately 9,600 adults from the NHANES and roughly 8,700 adults from the ARIC Study. The estimated prevalence rates of IDH based on 2017 ACC/AHA guidelines vs the JNC7 guidelines were 6.5% vs 1.3%, respectively. In a longitudinal analysis that included about 8,700 adults, the researchers did not identify any significant association between IDH as defined by the 2017 ACC/AHA guideline and incident atherosclerotic cardiovascular disease, heart failure, or chronic kidney disease. Thus, the researchers concluded that IDH, as defined by the 2017 ACC/AHA blood pressure guideline, may not be associated with increased risk for cardiovascular outcomes.
