Association of extreme nocturnal dipping with cardiovascular events strongly depends on age

Palatini P, Verdecchia P, Beilin LJ, et al. - Researchers conducted the first population-based study indicating that the risk of a future cardiovascular event in extreme dippers is heavily dependent on age. In this study, they analyzed 10,868 participants (53% men) aged 53±15 (mean±SD) years who were enrolled in 8 prospective studies. Outcomes suggest that extreme dipping is not correlated with an increased risk of a cardiovascular event in individuals younger than 70 years and hence it should be regarded as an innocuous condition in these individuals. Extreme dipping is identified to be a significant predictor of a cardiovascular event in individuals 70 years of age or older. The risk of cardiovascular event related to extreme dipping is 4× higher in octogenarians vs dippers; the risk in octogenarians is as high as that of reverse dippers. The present data indicate that the feature of dipping can significantly increase the prognostic capacity of established risk prediction models in individuals ≥ 70 years.
