Association of extreme nocturnal dipping with cardiovascular events strongly depends on age

Palatini P, Verdecchia P, Beilin LJ, et al. - Since there is a lack of clarity about the link between extreme dipping and cardiovascular events (CVE), researchers undertook this study with 10,868 participants (53% men) aged 53 ± 15 (mean ± SD) years enrolled in 8 prospective studies, to investigate if the prognostic role of extreme dipping varies as a function of age. Four groups were defined using the ambulatory systolic blood pressure nocturnal drop: dippers (> 10%–20%), nondippers (> 0%–10%), reverse dippers (≤ 0%), and extreme dippers (> 20%). A median follow-up of 5.7 years revealed 829 CVE (168 fatal). In individuals younger than 70 years, findings revealed no link between extreme dipping and poorer outcome. In participants older than 70 years, a U-shaped link between nocturnal blood pressure dipping and adverse consequence was revealed. In this study, the risk of CVEs was found to be 4× higher in the octogenarian extreme dippers as compared with the dippers and was similar to that in the reverse dippers.
