Association between use of primary prevention implantable cardioverter-defibrillators and mortality in patients with heart failure: A prospective propensity-score matched analysis from the Swedish Heart Failure Registry

Schrage B, Uijl A, Benson L, et al. - Researchers used a contemporary heart failure with reduced ejection fraction (HFrEF) cohort to determine the link between implantable cardioverter-defibrillator (ICD) use and all-cause mortality. They also studied relevant subgroups. Participants were selected from the Swedish HF Registry. Only those satisfying the European Society of Cardiology criteria for primary prevention ICD were included. Eligible patients were 16,702 in total, of those, only 1,599 (10%) had an ICD. Following propensity score matching, a comparison was performed between 1,305 ICD recipients and 1,305 non-recipients. Findings revealed underutilization of ICD for primary prevention in this contemporary HFrEF population, although reduced short- and long-term all-cause mortality was reported in relation to the use of ICD in this study sample. Among all the examined subgroups, this link was found to be consistent. The necessity for better implementation of ICD therapy was emphasized.
