Association between aspirin use and biliary tract cancer survival

JAMA Oncology Oct 24, 2019

Jackson SS, Pfeiffer RM, Liu Z, et al. - Given the proposal of using aspirin to reduce cancer mortality as it may slow cancer growth through the inhibition of both cyclooxygenase, which promotes inflammation and cell proliferation, and platelet aggregation, which may slow the metastatic spread of cancer, researchers examined if postdiagnosis aspirin use is associated with biliary tract cancers (BTCs) survival. They identified 2,934 eligible patients with BTC; of these 667 (23%) had GBC, 1,559 (53%) had cholangiocarcinoma, 224 (8%) had ampulla of Vater cancer (AVC), and 484 (16%) had overlapping cancer. Aspirin use was reported in 256 (9%) patients at baseline; there were additional 349 (12%) patients who initiated aspirin use after diagnosis. Postdiagnosis aspirin users across all BTC types exhibited a reduced risk of death. Platelet activation protects tumor cells from elimination, promotes metastatic cell growth, and facilitates cancerous cells to spread via the bloodstream. Aspirin via inhibition of platelet aggregation may slow the metastatic spread of cancer cells and improve BTC survival.
