Assessment of the German and Italian Stress Cardiomyopathy score for risk stratification for in-hospital complications in patients with Takotsubo syndrome

Santoro F, Gil IJN, Stiermaier T, et al. - Researchers derived a simple clinical score to anticipate risk of in-hospital complications among patients with Takotsubo syndrome (TTS), an acute, reversible heart failure syndrome featured by significant rates of in-hospital complications, through a multicenter prognostic study of 1,007 patients from the German and Italian Stress Cardiomyopathy (GEIST) registry and 946 patients from the Spanish Registry for Takotsubo Cardiomyopathy. The GEIST prognosis score was developed by granting 20 points each for masculine gender and history of neurologic disorders and 30 for right ventricular involvement and then subtracting the value in percent of left ventricular ejection fraction. Data reported that in-hospital complication rates were 12.7% for low-risk patients, 23.4% for intermediate-risk patients, and 58.8% for high-risk patients. Individuals with in-hospital complications had significantly higher rates of mortality than those without complications after 2.6 years of follow-up. In early risk stratification for TTS, the GEIST prognostic score may be beneficial. High-risk TTS patients may need an intensive care unit stay, and in a few days, low-risk TTS patients may be discharged. In-hospital complications in TTS patients may be linked to increased risk of long-term mortality.
