Anterior instrumentation through posterior approach in neglected congenital kyphosis: A novel technique and case series

Elnady B, et al. - Researchers analyzed 14 subjects with a mean age of 19.4 years between 2010 and 2014, to introduce a technique for anterior instrumentation and reconstruction with posterior-only vertebral column resection (PVCR) for the treatment of severe neglected congenital kyphosis through a posterior approach. They involved cases with severe congenital kyphosis more than 90°. They recorded 2.4 vertebrae per patient, the mean number of resected vertebrae; and 6.4 cm, mean height of the anterior defect after resection. The mean preoperative local kyphosis angle was observed 104.6° and improved to a mean of 22.8° at 2-year follow-up whereas improvement in sagittal vertical axis was noted from 62.7 mm preoperatively to 21.4 mm at 2-year follow-up. They concluded this technique a novel process for the preservation of high degrees of sagittal plane deformities.
