Algorithm-based reduction of inappropriate defibrillator shock: Results of the Inappropriate Shock Reduction wIth PARAD+ Rhythm DiScrimination–Implantable Cardioverter Defibrillator Study

Ruiz-Granell R, et al. - In a general population in whom implantation of a dual implantable cardioverter-defibrillator or a triple-chamber defibrillator (cardiac resynchronization therapy–defibrillator) featuring PARAD+ was used for primary or secondary prevention, researchers evaluated inappropriate shock (IS) reduction with the PARAD+ discrimination algorithm. A follow-up for a median of 552 days (interquartile range 354; 725) was performed in 1,013 patients (80.7% male; age 67.1 ± 11.4 years; 68%/30%/2% primary/secondary/other indication). According to the findings, in relation to the use of the enhanced PARAD+ discrimination algorithm alone, the annual rate of defibrillator patients with IS ranged from 1.0 to 2.1 per 100 person-years, so use of the enhanced discrimination algorithm PARAD+ led to a very low rate of patients with IS in a general population implanted for primary or secondary prevention.
