Alcohol septal ablation in patients with concomitant hypertrophic cardiomyopathy and aortic valvular stenosis

El-Sabawi B, Nishimura RA, Guerrero ME, et al. - In this case series of six patients with concomitant significant dynamic left ventricular outflow tract (LVOT) obstruction and aortic valvular stenosis (AS) who underwent alcohol septal ablation, researchers focused on the hemodynamic and early clinical results of percutaneous alcohol septal ablation. All the participants had severe dynamic LVOT obstruction (resting or provoked gradient ≥ 50 mmHg), mild to severe AS, and NYHA class III symptoms or greater. The procedure was performed successfully in all patients and was related to an immediate decrease of the dynamic LVOT gradient with a residual fixed obstruction associated with AS. Findings revealed the feasibility of alcohol septal ablation for relieving dynamic LVOT obstruction in patients with concomitant hypertrophic cardiomyopathy and AS.
