Additive value of high-density lipoprotein cholesterol and C-reactive protein level assessment for prediction of 2-year mortality after transcatheter aortic valve implantation

The American Journal of Cardiology Apr 11, 2020

Zieliński K, Kalińczuk L, Chmielak Z, et al. - Researchers analyzed 334 patients treated successfully with transcatheter aortic valve implantation (TAVI) who had complete 2-year follow-up, to assess the separate as well as combined prognostic values of baseline HDL-C and CRP levels in this group. Experts noted that 2-year mortality prediction was enabled by baseline HDL-C ≤ 46mg/dL (AUC = 0.657) and CRP ≥ 0.20mg/dL (AUC = 0.634). Following stratification with both cutoffs, LVEF ≤ 50% was most commonly seen in patients with low HDL-C and concomitant high CRP, and these patients were high-risk according to EuroSCORE II. The lowest frequency of LVEF ≤ 50% was noted in patients with isolated CRP rise, but these had more sarcopenia (based on psoas muscle area). Overall, the joint contribution of low baseline HDL-C and high CRP in the prediction of increased all-cause mortality post-TAVI was identified.
