Acute total hip arthroplasty combined with internal fixation for displaced acetabular fractures in the elderly: A short-term comparison with internal fixation alone after a minimum of two years
The Bone & Joint Journal Apr 04, 2019
Borg T, et al. - Given that a combination of open reduction and internal fixation (ORIF) and acute total hip arthroplasty (THA), an approach called ’combined hip procedure’ (CHP), has been advocated for displaced, comminuted acetabular fractures in the elderly, researchers compared the outcome after CHP or ORIF alone. Among a total of 27 patients with similar acetabular fractures (severe acetabular impaction with or without concomitant femoral head injury), 14 were treated with ORIF alone and 13 were treated with a CHP. The mean age of patients was 72.2 years (50 to 89) and follow up was done for a minimum of two years. Outcomes revealed considerably reduced need of further surgery among those who underwent CHP compared to those who underwent ORIF alone. In view of the findings, they support CHP use and its evaluation in larger prospective studies.
