A systematic review and meta-analysis of the safety and efficacy of endoscopic enucleation and non-enucleation procedures for benign prostatic enlargement

Wroclawski ML, Teles SB, Amaral BS, et al. - Different endoscopic enucleation of the prostate (EEP) techniques were evaluated for their safety and efficacy, via comparing laser (L-EEP) and non-laser (NL-EEP) procedures; and EEP vs other endoscopic non-enucleation (ENE) surgeries for benign prostatic enlargement (BPE). Researchers performed a systematic literature review and identified 35 RCTs (4,066 patients) for analysis; of these, 31 (3,909 patients) appraised EEP vs ENE, and 4 (327 patients) appraised L-EEP vs NL-EEP. As per outcomes, BPE could be safely and effectively treated with EEP and ENE. EEP was favored considering the perioperative data. EEP led to greater maximum flow rate. Also, EEP was correlated with less catheterization time, length of hospital stay, Hb decrease, transfusion rate. However, there was greater OT and bladder injury with EEP. They identified statistical differences, with questionable clinical significance in functional outcomes and complication rates. Greater tissue removal and smaller Hb decrease were noted in relation to L-EEP vs NL-EEP, with similar functional profiles.
