A meta-analysis of MitraClip combined with medical therapy vs medical therapy alone for treatment of mitral regurgitation in heart failure patients

Giannini C, et al. - Researchers intended to ascertain whether percutaneous mitral valve repair with the MitraClip yields superior survival outcomes than those seen with medical therapy in patients with functional mitral regurgitation (MR). They analyzed evidence from studies reporting MitraClip vs medical therapy with multivariate adjustment and with >80% of patients with functional MR. Overall 2,121 participants, 833 treated with MitraClip and 1,288 with conservative therapy, were analyzed overall in 6 eligible observational studies identified from PubMed, MEDLINE, and Google Scholar. These studies reported death from any cause as the primary endpoint, while freedom from readmission was the secondary one, evaluated with random effects. A significant survival benefit was seen with MitraClip vs conservative treatment. Among patients with functional MR and across all the main subgroups, this superiority was specifically pronounced.
