Early F, et al. - This study comprises a case series, wherein, the combined effects of an off-the-shelf Internet-based health-promotion program, The Preventive Plan (TPP), and nurse-coach support were assessed in COPD patients. This program was designed to increase patient activation and provide self-management benefits. Findings demonstrated that in COPD, combining nurse-coach support aligned to an Internet-based health resource, TPP, was feasible. Furthermore, data provide learning about the challenges of such an approach and the importance of the nurse-coach role.
