Smith PA, et al. - Researchers estimated the role of the anterolateral ligament (ALL) and the deep fibers of iliotibial band (dITB) for control of a simulated pivot shift compared with minimally invasive extra-articular anterolateral tendon graft reconstruction vs modified Lemaire reconstruction (MLR) after anterior cruciate ligament (ACL) reconstruction (ACLR). They noted the production of significantly more anterior translation at all flexion angles while sectioning of the dITB vs ALL (after ACL sectioning). A significantly more internal rotation at 0° vs ACL plus ALL was noted in the ACL-plus-dITB sectioned state. They observed the return of the knee in conjunction with ACLR to its native biomechanical state without causing over-constraint with ALR.
